I confirm all of this information is true and submitted by the Name entered above. I am willing and able to participate in this training and pay the $500 (200 hour) and $750 (300 hour) deposit within 5 days of acceptance. I acknowledge this training takes place at Samadhi Yoga- Englewood for 9 weekends over the course of April-August or April-September (300 hour) 2020.

Basic Student Agreement: I acknowledge the deposit paid is non-refundable. I agree to attend all weekends of training, or make-up and pay 50$ per make up hour (maximum 3 hours per day missed) and complete at home work in order to complete training successfully. I will teach a demo class and complete all assignments and readings to graduate.
